September 20, 1994 Steve Richey KHS, Inc. 1264 East Walnut St. Carson, CA 90746 (310) 632-3773 RE: Control Tech Road Stem Recall Notice Dear: Steve Richey, We have become aware of a potential safety hazard with Control Tech Road Stems and Control Tech A-Head Road Stems that could cause catastrophic failure and serious injury. Out of the approximately 28,000 Road stems shipped from 1992-1994, six road stems have been returned to Control Tech with the handle bar clamp detached from the remainder of the stem at the weld. Additionally, Control Tech has received two stems in which the bike owner has observed a crack in the weld at the handle bar clamp. Therefore, effectivelye immediately, we are conducting a voluntary recall of all Control Tech Road and A-Head Road stems. Until further notice, Control Tech is requesting that you temporarily discontinue all sales and shipments of road stems. We also ask that you provide your customers with a copy of this notice and request that you temporarily discontinue sale of all Control Tech road stems that have been in use, in order to ascertain the problem. We thank you for your cooperation and apologize for any inconvenience that this may cause. This notice applies only to 26.0 mm bar clamp diameter road stems and does not affect the Control Tech mountain bike stems.
